Finding all 50 Codex entries is an optional collection. The achievements page lists the public medal names associated with relic and secret discoveries, but the specific locations of individual QR codes are player-reported and may vary by route.
The Codex is the game’s primary lore delivery system for players who want to understand the world beyond the military briefing audio. Secret weapons in Acrophobia

Secret cache in the Spire (Long Haul)
The Long Haul achievement references a secret cache in the Spire. Verticality and scanning are the required skills. The Spire is a central landmark visible from many earlier chapters; Acrophobia is where it becomes the play space.
The Spire is the tallest structure in the Sprawl and appears in background shots throughout the campaign. Its appearance in Acrophobia as a playable space is one of the chapter’s set-piece moments. Players who have been scanning horizons throughout the game will recognize the space faster than players who have been focused purely on combat.
QR code tips
- QR codes are environmental, not hidden behind objects — scan rooms as you clear them. Look for small glowing objects on tables, shelves, and in corners.
- Some QR codes require alternate routes or backtracking after story progression unlocks new paths. If a door was locked in an earlier chapter, come back after major story events.
